💡 Model of Care
Operates an open-ended model with no discharge process. Clients can re-engage at any time without needing a new referral.
⏰ Location & Transport
No mileage restriction—individuals across the whole borough are welcome. Transport can be discussed at the triage stage.

Referrals can be made professionally or through self-referral.
