Home Blood Pressure Monitoring
Overview
For patients with a clinic BP greater than 140/90 (see the visual hypertension guideline for specific thresholds for different conditions), the next step should be home blood pressure monitoring.
Patient Has Their Own BP Machine
Action Required
Print the patient a BP diary to record their readings.
Options:
- Print from here
- Use the diary on SystmOne (Search HBPM bottom left of S1)
Patient Does Not Have a BP Machine
Referral Process
Send the patient to reception to ask to be put on the waiting list for a home BP diary.
Process Flow:
- Patient contacts reception
- Patient is placed on waiting list
- Patient is contacted and given a machine and diary
- Patient returns readings to be scanned
- A few weeks later, a GP reviews the readings
- GP arranges contact if action is required
