For patients with a clinic BP greater than 140/90 (see the visual hypertension guideline for specific thresholds for different conditions), the next step should be home blood pressure monitoring.

Print the patient a BP diary to record their readings.

Patient Does Not Have a BP Machine

Referral Process

Send the patient to reception to ask to be put on the waiting list for a home BP diary.

Process Flow:

1. Patient contacts reception
2. Patient is placed on waiting list
3. Patient is contacted and given a machine and diary
4. Patient returns readings to be scanned
5. A few weeks later, a GP reviews the readings
6. GP arranges contact if action is required
